Having decided that my HTML-y attempt at the form was actually more confusing than helpful, here's an easy version to copy-paste into an email or word processor and answer that way:

Slave Application Form

You can use this form to apply for a position as a BDSM "Bottom" with me. The form will help me to understand your approach to BDSM and whether or not you and I are well matched.


Please note, this form is NOT intended to be any kind of slave contract form, and is not intended to indicate or prescribe the eventual form of a relationship (although you could consider it to be a form of petition). Nor is it a BDSM checklist, although if your application is successful it may be helpful for you to be prepared to fill one out. This form is just to get an idea of who you are, where you're going, and what you might be expecting.

Part 5: More Detailed Information

This section is your chance to impress me with your individuality, your use of language, and letting me see the real you. Please use as much of the space provided with each of the following tasks. If it seems like you have already given some of the information in the earlier parts, please use this space to expand on those answers and be creative.

1/. I would like you to write about something that interests you outside of slavery. This can be anything you like, perhaps politics, or music, or books, your favourite sports, what you like to eat or drink. If you have space, please also list some other things that interest you, maybe with a few words about each.


2/. I would like you to tell me about whatever you feel your greatest achievement has been in your life so far. Tell me what it was, and why that particular thing stands out for you. It might be something to do with your (desire for) submission, or it might be something to do with your non-BDSM life.


3/. I would like you to write down any talents that you have, whether you think of them as being BDSM-related or not. Please be inventive in your answer, but not deceptive.


4/. I would like you to tell me about the things you enjoy (or fantasise about) most, about being a slave. For example, do you enjoy being controlled, do you enjoy being beaten or tortured, or maybe you enjoy focusing on someone else instead of yourself? Maybe there are particular sorts of service, or of pain, that you like more than the others - I would like to know what these are.


5/. Finally, would like to know about your hard-limits. I will not a statement that you have no hard-limits, because everyone draws the line somewhere. Even if you believe that you will accept anything from a master, I want you to tell me the things that you would find hardest to accept.


6/. Why are you applying for this position?

This is the most important question on the form. Please use this space to explain why you wish to be my slave in particular. You may say why you chose to fill in this form, you may talk about how well you feel you match my requirements and I match yours, or anything you feel may convince me that I should develop a relationship with you.
